How to Prepare for your Follow-Up Appointment
- Please bring
- The patient’s valid Health Card
- Please inform us of:
- Any updates to your contact information (address, phone number(s), etc)
- Any changes to physicians/specialists/services involved in the care of the patient
- Any changes to medications that the patient is taking
- Please be prepared to answer
- Updates on your child’s condition
- Seizure log (if applicable)
- Headache diary (if applicable)
- Updates on your child’s condition
Important Reminders
- Please be ON TIME. Our doctors work hard to keep their schedules on time for your convenience. Please be respectful of their time and ensure that you arrive at least 15 minutes before your scheduled start time.
- If you are going to be late please call the clinic to let us know. Please do not wait for us to call you after your appointment time has started.
- The location of your appointment
- The majority of appointments are held at the Pediatric Neurology Clinic on Argus Road (see ‘Contact Us’ for address)
- Occasionally, follow up appointments can be arranged to take place via Teleconference (via Ontario Teleconference Network) Please keep in mind that a teleconference via OTN (Ontario Teleconferencing Network) is NOT the same thing as a phone call. An OTN is a computer/video call that requires you to sign in to a link (that would have been emailed/provided ahead of time). If you DID NOT receive a link for your upcoming OTN teleconference, INFORM the CLINIC ahead of time, prior to your appointment. Please do not wait until the time of your appointment to inform us that you did not receive a link
*Ultimately, whether or not an appointment can be conducted via teleconference is left up to the Doctor’s discretion, and may not always be possible based on the nature of the visit. At times, an IN CLINIC visit is necessary. If the doctor has requested that an appointment be conducted in person in clinic, kindly respect this. Not all appointments can continue to be held virtually.
- *Please be advised that failure to show up to your appointment (no-show), or canceling/re-scheduling your appointment without giving us 48h (2 business days) notice results in a $50 fee that is not covered by OHIP
